Enhancement Coming Your Way in October

Coventry Health Care is transitioning your employer sponsored group health plan to a new claims and administration system in late October. These changes will improve our ability to serve you and will enhance your experience as a Coventry member. We are pleased to tell you about some of the enhancements you will notice as we make this transition.

This change does not include the following groups: Rural Carrier Benefit Plan or Foreign Service Benefit Plan.

Identification (ID) Cards

In October a new Coventry ID card will be issued to you and each of your covered dependents. The new card(s) will include updated information, including new:

  • Coventry member ID number and group number
  • address to submit claims
  • prior authorization/pre-certification telephone number

Your Coventry Customer Service Team and their telephone number will not change.

For members living outside of Nebraska, Iowa and South Dakota

Explanation of Benefits (EOB) and Letters

EOBs and letters generated after the transition date will have a new look. Click here for a sample of the new Coventry EOB with instructions on how to read it.


New Claim Form

For Coventry members using out-of-network providers, a new medical claim form will be required for claims submitted after October for claims where the provider is not submitting the claim on your behalf. Click here for a copy of the new Coventry medical claim form.


New Process for Dependents Living Outside the Service Area

If you are in an Health Maintenance Organization (HMO) or Point-of-Service (POS) health plan and your covered dependent child moves outside Nebraska, Iowa or South Dakota after October, you can enroll your dependent in Coventry's Passport program. Passport is designed to ensure your dependent gets the highest level of benefits when care is obtained from a provider in our Coventry National Network.

To enroll your dependent Passport program, please complete a Passport form and submit it to the address on the bottom of the form. Click here for a Passport form.


Secure Member Web Site

In late October you will have access to Coventry's enhanced member web site that features My Online Services. This secure member web site gives you access to a member portal with more information than ever, including:

  • a Personal Health Records tool - allow you access to pharmacy data, lab test results, medical claims and member-entered personal health information
  • Coventry WellBeing - a wealth of tools to help you live well
  • improved cost and quality tools to help you make wise choices about your health care
  • an ID card feature allowing you to print an ID card or request a new one
  • wellness reminders sent to you via e-mail
